At Gold Coast Health, a re-entry care pathway for patients discharged after prostatectomy and nephrectomy has been established.

These patients require ongoing surveillance and care for many years after surgery which is best managed by their GP.

When specialist review determines that your patient is suitable for this pathway a letter containing the follow up requirements will be sent to both the GP and patient.

Contact details for the service, including a mobile phone number for Urology Clinical Nurse Consultant, will be included in the letter.

GPs can contact this number for any queries regarding follow up of these patients or to highlight abnormalities detected on surveillance.
